Awards / Survey Plugin

On the site we run we do yearly awards. I was just wondering of the posibility of a new plugin for such a thing. It could be used for awards and also surveys.

The idea would be that the user would need to be logged in, or possibly it would only allow one submission per IP address.

Then it would give them a set of questions to answer which they submit and then goes to a database. I know this is similar to the form builder, but we could really use (and am sure others would benefit) from one that only allowed one submission per person.

What are your thoughts on such a plugin?

I know this is an old thread but what exactly do users want? I may consider coding it

I'd have to think about it myself... for my personal users I could use a contest type scenario where an artist submits an image and the rest of the user population votes on the image in certain ways.
