Portfolio Plugin?

I am looking for some one who can create a Portfolio Plugin. I am willing to pay a token amount for the efforts ;)

Here is what I am looking for:

1. Member should be able to submit his port folio entry with following "fields"

Name:
Contact:
Website:
About: Description
Image: Should be able to upload ONE image
Technical Details: about the image

2. Once submitted it should go to the Admin for approval before publishing on the site.

3. Would prefer if an automatic email could be sent out about approval

4. Should be able to categorize entries based on month
