Support Desk Plugin Mod

I am looking for a mod to the support desk.

Specifically, I'd like to ability to have custom fields (like the form builder) on the support form so I can ask for specific information.

I was coming over to ask for the EXACT same thing.

In addition to custom fields, I need the ability to upload attachments.
